As participants go through different stages of life, Evergreen Care and Support workers will provide mentoring, peer-support and development of skills to enable an easy transition. We apply person-centred approach in assisting participants in meeting their needs and achieving their goals. This may involve
support with shopping, paying utility bills, daily planning, applying for rental tenancy, understanding and maintaining budget, and enhancing their strengths to participate in their community.
Evergreen Care and Support workers will offer hygiene and personal care assistance to participants in their homes. Depending on individual's needs, our well-trained and respectful staff can assist participants with showering, toileting, changing and related personal hygiene activities.
This support aims to develop the participant’s daily life skills to
help them uphold independence.
Evergreen Care and Support will assist participants whose disability makes it difficult to access transport. We provide service that enables participants to attend any
activities outside of the home, such as: to and from appointments, recreational and social community outings,
or a workplace..
Depending on the support level, a worker can transport a participant, accompany a participant to achieve their travel needs through public transport or their own vehicle. Participants can also choose to develop skills in using public transport independently.
When required, specialised disability transportation will be hired to facilitate travel to an educational facility or
community destination.
Evergreen Care and Support has a Registered Nurse to deliver specialist nursing care for participants who have care support needs. We intend to hire more nurses in the next phase of growth. Our policy ensures relevant training is undertaken by staff so that they can best respond to each participant’s specific and/or complex care needs.
Our workers will assist with participant’s' home
and personal care in a Specialist Disability Accommodation (SDA)
and shared living arrangements. We offer Supported Independent
Living which
focuses on skill development and self-determination
relating to the unique needs of each individual living in a shared
arrangement. As a provider, our responsibilities will include supervision
or support with: personal care; personal hygiene; mobility; and assistance with attending appointments.
Evergreen Care and Support workers will focus on training the client to develop skills that can increase their ability to live independently, as well as participating in community activities. Such activities may include personal care and hygiene training, shopping for groceries and planning meals, perform household tasks such as clothes-washing/laundry, general house cleaning and vacuuming, planning public transport journeys, enhancing decision-making skills and developing problem-solving skills.
Evergreen Care and Support workers will support or supervise a participant to take part in community, social,
recreational or economic activities. This is provided in a range of
environments, such as: the local community centre; social and educational hub, leisure and recreational centres. movies and shopping trips to shopping centres of their choice.
Assisting participants in developing, maintaining and strengthening social ties in their community will improve their psychological and emotional well-being.
